This website is intended to collect general business inquiries only. Do not submit patient information, personal health information, confidential facility details, credentials, interface messages containing patient identifiers, or protected production data through the contact form.
Information collected
The contact form may collect your name, email address, organization, role, product interest, message, submission time, and IP address. In this starter website package, form submissions are saved to data/leads.csv on the server.
Use of information
Information submitted through the form is used to respond to inquiries, discuss demos, and evaluate product fit.
Production recommendation
Before launch, replace CSV storage with a secure CRM, database, or email workflow approved by your hosting, privacy, and security requirements. Add spam protection, HTTPS, retention rules, and a formal privacy policy.
